Responsibilities: As a Team Coordinator at Snowline Hospice, you will be responsible for providing compassionate and comprehensive nursing care to our patients and their families. Your duties will include:

  • Helping to maintain excellent collaboration and communication within the team and the organization
  • In close collaboration with other disciplinary teams, you will be actively supporting quality care and assist in improving the lives of others
  • Help support team in reviewing daily tasks, visits, and patient needs
  • Provide direct administrative support in the handling of duties and functions related to clinical operations for the organization
  • Helping people through difficult times and providing support to patients or families and friends when needed
  • Enjoy meaningful and friendly interactions as you join forces to make good things happen
  • Prepare and coordinate clinical work schedules and calendars
  • Receive incoming patient related phone calls and coordinates with physicians, families/caregivers, and staff members
  • Track team information including time worked and time off requests
  • Clinical data collection, metrics, and reporting
  • Supports daily administrative functions including reporting, meeting coordination, orientation, training, safety updates, working closely with IT support, and conducting reviews
  • May provide support for other administrative tasks or team members when appropriate
  • Calm, soothe and empathize with the strong emotions our patients and families are experiencing
  • Motivate and encourage a team of healthcare professionals in providing the best care possible to each patient
  • Assist with the management of clinical notes, health records, and systems to ensure appropriate and accurate data

Requirements

Our ideal candidate.

  • Associates degree, Bachelors degree or combination of professional experience and college course work preferred
  • Four years general office and administrative support experience OR two years general office and administrative support plus an associate degree in business or related field
  • One year experience in a healthcare setting, preferably with scheduling experience
  • Demonstrated ability to work independently, managing multiple projects while performing work in a timely, accurate, and confidential manner
  • Proven ability to think clearly, logically, analyze issues and use good judgment to carry out responsibilities
  • Demonstrated computer skills with extensive experience using Microsoft Word and Excel required
  • EHR (Electronic Health/Medical Record) experience preferred
  • Excellent written and verbal communication skills and ability to work harmoniously and cooperatively with a wide variety of people
  • Embraces different belief systems and respects individual choices and lifestyles
  • Excellent observation, problem solving, verbal and written communication skills
  • Demonstrates positive behavior characteristics such as attitude, initiative, cooperation, adaptability, fairness, ingenuity, and dependability
  • Handle confidential information in an appropriate manner and uses discretion when handling sensitive information
  • Valid CA driver’s license, with a reliable vehicle that is insured in accordance with state requirements
  • Is open to occasional weekend or evening work
