Qualifications: previous experience in hospice, home health

Current LVN license in California.

• Minimum one year experience in Hospice or Home Health.
• Demonstrated understanding of Hospice philosophy.
• Excellent written and verbal communication skills.
• Valid driver's license, auto insurance, and reliable vehicle.
• Must be able and willing to travel locally within assigned territory.

Responsibilities: Implement current nursing practice following a comprehensive assessment and the plan of care

Accurate and timely documentation of patient services reflecting the plan of care

Assess & provide patient and family/care giver the education and information pertinent to diagnosis and self plan of care

Report appropriately any identified needs to the interdisciplinary group

Examples: hospice aid, OT, PT, MSW, ST, Dietitian or Clinical Supervisor / Nursing Supervisor

Provides appropriate pain/symptom management

Properly evaluates patients response to treatments/ medications

Job Description :

Socal Premium Hospice Care is seeking Part Time and/or Per Diem compassionate and responsible Licensed Vocational Nurse. Under the direction of a Registered nurse, the Licensed Vocation Nurse provides skilled nursing for the assigned hospice patients. He/she works cooperatively with the other members of the Interdisciplinary Hospice Team. The LVN provides intermittent visits or shifts of skilled nursing care, performs prescribed medical treatments, including pain management and symptom control, provides nursing observations, provides teaching regarding the expected course of the disease, care techniques and other health measures to the patient, family or other primary care giver, and participates in the care plan development and implementation.
