Description

Director of Social Worker needed for full-time, temp to hire position in Camden County.
Oversee all aspects of Social Work Department operations within the nursing center, ensuring compliance with regulations. Advocate for resident needs, coordinate with interdepartmental staff, and manage team recruitment and supervision. Maintain relationships with external care providers. Prepare for state surveys and contribute to center meetings.
Key Responsibilities:
- Ensure thorough documentation of residents' psychosocial needs and compliance with regulations.
- Provide reports and recommendations to the Administrator.
- Coordinate MDS sections completion and discharge planning.
- Facilitate the development and implementation of interdisciplinary care plans.
- Serve as Grievance Officer and manage complaint resolution.
- Ensure compliance with Advance Directives and health care decision-making processes.
- Facilitate resident transfers and adjustments.
- Participate in QAPI and perform regular audits.
- Maintain resident psychological services list and pertinent documentation.
Position Requirements

NJ CSW, NJ MSW preferred.
