Healthcare Navigator/Social Worker needed for full-time, temp to hire position in Montgomery County.
We seek an experienced Healthcare Navigator/Social Worker to counsel patients and caregivers with rare neurological conditions. The ideal candidate is a self-starting problem solver with expertise in social work.
Key Responsibilities:

• Insurance Guidance: Assist with private and public healthcare insurance reimbursement.
• Patient Resources: Provide one-on-one consulting on psychosocial and financial resources, social security disability, adaptive devices, transfer of care, discharge planning, and rehabilitative care.
• Community Groups: Lead online support groups for various community members.
• Outreach & DEI Initiatives: Represent the Foundation at 3-4 health fairs annually, focusing on remote or underserved populations.

Position Requirements

PA LSW or PA LCSW - Minimum 7 years of experience in a healthcare or hospital setting, preferably with chronic illness populations. Infusion center experience and bilingual (Spanish/English) are pluses. Strong problem-solving skills and independent work capability.
