Description

LTC/Subacute Social Worker needed for full-time, temporary (3 month) position in Burlington County.
The Social Worker will be responsible for providing direct clinical social work services to residents and their representatives in a multidisciplinary team approach. The Social Worker plans, coordinates or assists with the facility placement, adjustment and discharge planning and coordination of community resources.
Position Requirements

NJ LSW with long term and subacute experience and is proficient with PointClickCare EMR.
