Work as a team with the RN/CM, caring for hospice patients in homes and facilities. Attends IDG meetings. Excellent communication skills to be used with the patients, families and caregivers. Compassion and professionalism toward the patients and families, and caregivers. This is for the Central/East Valley

Must have a current Arizona RN license, dependable transportation, current TB.
