Position Summary:
We are seeking a dedicated and passionate Inclusion Infusion Pharmacist to join our team in Cumberland, Maryland. The Inclusion Infusion Pharmacist will play a pivotal role in ensuring that our infusion services are delivered with a focus on inclusivity, accessibility, and cultural petency. This position offers a unique opportunity to contribute to the enhancement of healthcare services for diverse patient populations.
Duties and Responsibilities:

• Collaborate with healthcare providers to develop culturally petent infusion therapy plans tailored to individual patient needs.
• Provide pharmaceutical care and medication management for patients receiving infusion therapy.
• Conduct patient assessments to identify barriers to care and develop strategies to address them.
• Advocate for the needs of underrepresented munities and ensure that healthcare services are accessible to all patients.
• Participate in munity outreach initiatives to promote health education and awareness.
• Stay updated on best practices, guidelines, and regulations related to infusion therapy and cultural petency in healthcare.
• Maintain accurate patient records and documentation in pliance with regulatory standards.

Requirements:

• Doctor of Pharmacy (Pharm.D.) degree from an accredited institution.
• Active pharmacist license in the state of Maryland.
• Minimum of 1years of experience in infusion pharmacy practice.
• Strong understanding of cultural petency and its importance in healthcare delivery.
• Excellent munication and interpersonal skills.
• Ability to work collaboratively in a multidisciplinary team environment.
