• Schedule: Monday - Friday 8:30am - 4:00pm
• Hourly Pay Rate: $22

Requirements:

• Bachelor in Social Work , Health Education, Public Health or related field
• (2) years of experience in public health education materials, and workshops on HIV/AIDS
• Bilingual English/Spanish

Responsibilities:

• Provides direct service to patients selecting service types compatible with assessment and identified adherence goals
• Identifies, selects, accesses, and makes referrals to appropriate community resources
• Participates in case conferences, staff meetings
• Maintains appropriate documentation as defined by hospital policies and procedures and program requirements
• Sets appropriate priorities and responds to follow-up situations in acceptable time period
• Demonstrates ability to make decisions, solve clinical problems and manage resources
• Demonstrates knowledge of HIV treatment
• Assists in data collection efforts for evaluation and research purposes
• Retrieves CLIMACS data on patients as needed"

Job Types: Full-time, Contract

Pay: $22.00 per hour
