Position: ​Licensed Master Social Worker ​
Dept: Inpatient/Discharge Planning
Location: Bronx & Manhattan
Schedule: 5x8 with Rotating weekends - 8am - 4pm
Hourly Pay: $65

Overview: LMSW will Asses patients' needs, provide counseling and work with agencies or insurance companies to cover the costs of treatment and medication after discharge.

Requirements:

• Active NYS LMSW or LCSW
• Master's Degree in Social work or related field
• Proof Of Full Covid-19 Vaccination
• 2 experience providing inpatient (Hospital or clinic) care coordination & discharge planning
• Ability to work every other weekend as needed

Responsibilities:

• Physically meet the members where they are to gain a deep understanding of their situation and needs
• ​Conduct discharge planning and members' ​needs: clinical, psychosocial, financial, environmental
• Prepare a member-oriented plan of care with members, caregivers, and health care providers
• Insure member caregiver understanding as it relates to language barriers, stress reaction or cognitive limitations/barriers using verbal and nonverbal techniques
• Advocate for members by assisting them to address challenges, and make informed choices regarding clinical status and treatment options
• Maintain accurate, comprehensive and current clinical and non-clinical documents

Job Types: Full-time, Contract

Pay: From $65.00 per hour

Expected hours: 40 per week
