The role:

The Clinical Case Manager is responsible for working with the clinical services department at the Villages and Communities to best support clinical programming and the youth and families served via those programs.

Responsibilities:

  • Assist in the development and implementation of program and services that meet the needs of our clients.
  • Coordinate and Monitor services for youth/family including educational and mental health services.
  • Facilitate access via referrals and linkages to community resources and support systems for youth and their immediate family.
  • Advocate for youth/family needs and rights within various service systems.
  • Conduct educational workshops to support parental involvement.
  • Provide training and support to staff and volunteers as needed.
  • Participate in multidisciplinary team meetings.
  • Work closely with other professionals specifically school personnel, social workers, therapists or other identified members of the care team.
  • Ensure compliance with all relevant policies, regulations and ethical standards.
  • Prepare reports to summarize data collection.
  • Maintain detail and accurate case records, notes and other documentation.
  • Participate in ongoing professional development and training opportunities.
  • Participate in community outreach activities to raise awareness to available resources.
  • May conduct home visits to help parents support their child's learning and mental health needs.

Qualifications:

  • Bachelor's Degree in Social Work, Psychology, Human Services or Education field OR any combination of education, training and experience which demonstrates that the applicant is likely to possess the required skills, knowledge, or abilities, may be considered
  • Valid Illinois driver's license with an acceptable driving record and insured and reliable transportation.
  • Ability to pass the required DCFS criminal background check, including child abuse and sexual offender registry.
  • Ability to work with DCFS, courts, and other social service systems.
  • Strong understanding of child development, family dynamics and community resources.
  • Previous experience in case management, social services or a related field.
  • Excellent communication, problem-solving and organizational skills.
  • Ability to work independently and as part of a team.
  • Proficient in Microsoft Office products.
  • Sensitivity to the cultural and socioeconomic characteristics of clients and staff.
  • Ability to relate to the clients and staff and to the public in a courteous and professional manner.
  • Reliable, punctual, and able to manage multiple tasks.
  • Ability to work in a tobacco, alcohol, drug, and firearm-free environment.
  • Ability to exercise discretion and judgment and to handle sensitive and confidential materials.
  • Ability to work flexible hours, to include weekends.
  • Commitment to One Family philosophy and mission.
  • Able to lift 50lbs continuously.
  • Able to walk up and down 3 flights of stairs without issue.
