South Davis Community Hospital is hiring a Nurse Case Manager to work on our hospital campus (32-40 hours/week). As a Case Manager, you will be responsible for coordinating patient care and providing support to individuals in need of medical services. Your role will involve collaborating with healthcare professionals to ensure the best possible outcomes for patients.
Prior nursing experience required, case management experience preferred. The Nurse Case Manager works closely with the admissions and marketing team to coordinate patient admissions and continued stay. The successful candidate will be responsible to review requests for initial authorization, continued stay, specific medications, or treatments. Your duties include but are not limited to reviewing medical records and documents related to the application.
Minimum Qualifications:
Must hold a current state RN/LPN license in the State of Utah
Long term acute care, Hospital, SNF, or short term rehabilitation experience preferred
Exceptional time management and communication skills
Basic Job Duties & Knowledge Base:
Demonstrated team leadership, relationship building, critical analysis, and written and verbal communication skills
Knowledge of funding resources and clinical standards and outcomes
Demonstrated competent, independent judgment to assess and meet client needs
Case management services include monitoring patient care to ensure progress toward desired outcome, addressing patient and family needs, resolving obstacles to effective care, coordinating care with payers and vendors, patients and families
Together with the multiple other internal team member the case manager is responsible for establishing, monitoring and executing patients discharge plan
Coordinates case management process from patient's entry into the healthcare system to discharge
Coordinates care and resources with physicians, social workers, and other team members to achieve optimal patient outcomes
Facilitates cost effective outcomes by determining appropriate level of care based on diagnosis, severity, intensity of services required, and other relevant criteria, using national and regional length of stay standards and community norms
As a Case Manager you will be joining a collaborative team that is dedicated to serving others in need and also supporting one another
Be responsible for promoting patient-centered care by coordinating the plan of care for the patient stay and managing the length of stay
