Southern Grace Hospice is Hiring a Medical Social Worker Near Mc Donough, GA

Job Description

Primary function is to provide coordinated care in the home to patients of all age groups and perform psycho-social assessments, analysis, counseling and referrals to meet the needs of the patient/family. The Social Worker will perform their duties in accordance with the mission of the Hospice; they will strive to provide quality care and kindness to all patients, families, volunteers and other staff members.

Duties include but are not limited to:

1. Completes a Psychosocial Assessment of each hospice patient; communicates this information to the team servicing the patient and during the weekly IDG meetings. These assessments are completed within 5 working days of the patient’s admission.
2. Makes visits to the patient/family in order to interview the patient and complete a psycho-social assessment. Visits will be made to the patient’s home, hospital and/or nursing home where they reside. These visits will be timely and in accord with the Patient Care Plan.
3. Places timely documentation notes in the patient’s medical record in accordance with the social work standard process and procedure. Documentation will follow the Problem Orientated Charting method and will be comprehensive of the patient’s psychosocial situation. Participates in the development of the Patient Care Plan; reflects problems, goals, interventions and expected outcomes.
4. Attends IDG and contributes to the patient discussion by identifying significant data that will enable the team to identify interventions that will enable the patient/family to achieve their goals.
5. Interprets the Hospice program to clients, families and/or other significant others. This includes communication with appropriate community liaisons.
6. Participates in the Orientation program for the new Volunteers organized by the Volunteer Coordinator.
7. Participates in chart audits as conducted by the agency; actively participates in Performance Improvement Activities of the Department and/or the Agency.
8. Counsels patient/family/caregivers as needed in relationship to stress and any other identified coping difficulty.
9. Provides information and linkage to appropriate community resources and/or agencies. When appropriate, serves as a liaison between the patient/family and the involved community agency/resource.
10. Assists physicians and other team member in understanding significant social and emotional factors related to health problems and/or death/dying issues.
11. Collaborates with the patient/family and home care team nurse in order to identify appropriate community resources that are necessary to stabilize the patient’s care at times of discharge from the hospice program.
12. Participates in community educational programs as requested.
13. Participates in monthly on-call psychosocial program as needed.
14. Participates in Quality Improvement activities as assigned.
15. Performs other duties assigned by the Clinical Managers.

Qualifications
1. Masters Degree from accredited School of Social Work. Licensed in the State of Georgia.
2. Prior Hospice experience preferred and/or related job experiences involving aspects of death and dying one year health or other human services experience will be considered.
3. Understanding of the hospice philosophy and the issues of death and dying.
4. Demonstrates good verbal and written communication skills. Possess good organizational skills and has ability to prioritize tasks and needs of patients and/or families.
