Southern IL Case Coordination Services provides conflict free case management to individuals who have a developmental disability .
The duties include but are not limited to :
- Development of a comprehensive Personal Plan
- Collaborating with service providers to ensure that the individual's health, safety, welfare and well-being is as desired and that the individual is satisfied with services received.
- The ISC encourages individuals and families to make informed decisions, exercise choice, and have maximum control over their lives.
- The ISC works from his / her home and is independent in scheduling required work duties and responsibilities.
Candidates must possess a Bachelor's Degree and a minimum of 1 year experience working with individuals who have a developmental disability .
An RN with a minimum of 1 year experience working with individuals who have a developmental disability.

Job Type : Full-time exempt status. No over-time is available.

- Remote office
- On the road (visits to homes, providers, employment) Standard IRS rate for mileage reimbursement
