Tailored Care Manager - Qualified Professional (QP)

As our full-time Care Manager for our Tailored Care Management Team, your primary role is to support and coordinate the mental and physical health care of identified members in need of TCM services. You will work closely with the TCM Supervisor and TCM Peer Support Specialist to ensure a fully-supported work environment.

The Tailored Care Management model reflects the goal of whole-person care management. Provider-based care management promotes integrated care and offers beneficiaries choice in how they receive care management. SouthLight is one of the first to offer this holistic approach to treating our community in need.

Pay

Starting pay is $24 per hour.

Schedule

Flexible 40hrs/wk, typically 9a-5p.

Full-time employees are eligible for medical benefits the first of the month 30 days after hire date. Options for a standard plan and an HSA plan with a $300 initial contribution and up to a $25 per paycheck match. 10 paid holidays and PTO hours which accrue at a rate of 10 hours per month. After 6 months employees are eligible to enroll in our 401(k) plan which has a 4% match. Company paid life insurance, short and long term disability, in house and paid training opportunities, and additional pay for working holidays. SouthLight is also an approved employer for the Public Service Loan Forgiveness Program (PSLF).

We are excited to offer an additional benefit of Clinical Supervision Reimbursement. We will provide a monthly reimbursement of up to $200 (full-time) or $100 (part-time) to each associate level clinician electing this benefit for no more than 2 years, or until licensure is completed, whichever is first. Licensures eligible to receive this benefit include LCSWA, LCMHCA, LMFTA, LPA, LCASA, and CADC.

Job Responsibilities of a Tailored Care Manager Qualified Professional

  • Screen and assess members for common mental health and substance use disorders
  • Facilitate member engagement and follow-up care
  • Provide member education about common mental health and substance use disorders and the available treatment options
  • Implement appropriate interventions to members under the supervision of the TCM Supervisor
  • Provide case management services as necessary
  • Provide assertive engagement to appropriate members
  • Complete documentation in a timely manner

Qualifications of a Tailored Care Manager Qualified Professional

Educational and Experience

  • Min of 3 years' experience providing case management or care coordination with adult MH/SU population in a community setting
  • Min of 2 years' experience working directly with adults struggling with mental health and substance use disorders

Licensure/Certifications

  • QP status required as specified in NC statue
  • Valid driver's license with clean driving record

Knowledge, Skills and Abilities

  • Ability to develop rapport with members and other professionals in a variety of settings
  • Ability to set firm boundaries with members
  • Excellent clinical skills, including evidence-based practices, diagnostic information (DSM 5), substance use intervention
  • Proficient in Microsoft Office Suite, Outlook, and electronic medical records
  • Excellent interpersonal skills and ability to work collaboratively within a team
  • Comfortable working independently in the community
  • Excellent oral and written communication
  • Ability to manage crisis situations both in person and over the phone
  • HIPAA and Substance use specific confidentiality and privacy rights
