Southwest Center is Hiring a Direct Support Staff Nurse (RN or LPN) Near Louisville, KY

We are seeking applicants for a Direct Support Staff Nurse is an RN/LPN who interacts with participants, staff, caregivers, physicians, families, behavior specialists, case managers, and other designated team members for a participant to ensure that the health/medical needs of the participants are met daily. The Staff Nurse will work with the RN Manager to ensure our participants' health, welfare, and safety. Qualifications/Requirements:
Must be a licensed as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) with the Kentucky Board of Nursing
Must have one (1) year of nursing experience working with adults
Experience in working with children or adults with intellectual or developmental disabilities, preferred.
Must be 18 years of age
Must pass a criminal background check
Must pass pre-employment & random drug testing
Must have a valid driver’s license and good driving and good driving record
Must have excellent communication, and organization skills
Ability to maintain client confidentiality
Must be proficient in MS Office Suite for document creation, editing and organization
Ability to multitask in a busy work environment.
Must have basic knowledge of typical office equipment & technology
Responsibilities:
Participates in the Monthly Incident Report Committee meetings to assist in the evaluation of Incident Reports as directed.
Interacts with participants, staff, caregivers, physicians, families, behavior specialists, case managers, and other designated team members for a participant to ensure that the health/medical needs of the participants are met on a daily basis.
Assist with ongoing assessment of participants as needed including those that are served through the Residential program.
Communicates with physicians and assists in securing signed orders as needed.
Assists the Medical Team to evaluate all Medication Administration Records (MAR’s) weekly to ensure all information is current and no med errors have occurred as directed by RN Manager.
Assists the RN Manager in evaluating med errors and developing plans to reduce the occurrences
Rotates medical back-up on-call with the RN Manager in the event a participant goes to the ER and to answer medical concerns reported by the DSP’s for the participants of the residential program.
Assists in monitoring the specialized diets as ordered by physicians.
Participates in the Quality improvement Program of the SWC.
Contacts medical staffs, and representatives of outside agencies or groups, participants, families, case managers, as needed.
Keeps professional license and CEU’s current as directed by the KY Board of Nursing
Cooperates and works as a team member with co-workers and supervisors. Treats co-workers and supervisors with respect. Promotes teamwork and maintains a positive attitude.
Completes initial and annual training-(CPR, First Aid, Tb skin assessment, etc.) as directed by Agency policy and regulatory standards. Attends monthly staff meetings and additional staff development trainings as scheduled.
Adheres to Agency policy and procedures. Maintains a consistent work attendance record. Limits use of calling off to a minimum and notifies direct supervisor in appropriate amount of time when unable to report to work.
Fosters collaborative working relationships among members of the leadership and management teams and programs.

1:00pm to 9:00pm or 2:00pm to 10:00pm or 3:00pm to 11:00pm
Shifts may include 1st, 2nd, 3rd, weekends, and holidays
Ability to commute/relocate: Louisville, KY 40258: Reliably commute or planning to relocate before starting work (Preferred)
