Hillcrest Youth Services is currently seeking a Full-Time Case Manager to work as part of an interdisciplinary team focused on coordination of services provided to residents to ensure excellent care. Compensation for this position is $19.00 an hour.
ESSENTIAL FUNCTIONS RESIDENTIAL CASE MANAGER:
  • Collect, compile, and disseminate necessary information on all referrals and admissions to Hillcrest staff when needed.
  • Assist in developing initial treatment plan and goals upon admission to program
  • Orientate new residents to rules, procedures, and general residential programs and services.
  • Coordinate visitation for residents with approved family members.
  • Maintain communication with family members concerning resident appointments as required.
  • Attend assessments with residents as needed.
  • Oversee residents, including approval of visits and phone contacts.
  • Maintain approved contact lists.
  • Oversee the needs of the youth while ensuring follow-up and information is communicated as appropriate.
  • Coordination of after care services.
  • Maintain communication with the referral agencies to assess community needs, trends in placement, and other relevant information.
  • Assist referral source/agency in identifying other appropriate agencies or services if admission to Hillcrest is denied.
  • Assist with coordination of treatment plan goals and resident's needs with outside agencies and family members involved in the treatment of the resident.
  • Maintain appropriate documentation.
  • Provide family education.
  • Weekly updates to placing agencies.
  • Monthly program reports to placing agencies.
  • Monitor resident's grades on an ongoing basis and provide a printout monthly for the medical record if Educational Coordinator is out.
  • Represent Hillcrest in an appropriate manner while at school or court.
  • Maintain required twenty hours per year of ongoing training.
  • Participate in treatment team meetings.
  • Adhere to Hillcrest's Quality Care Standards.
  • Provide consultation/education services as needed to other Hillcrest staff and outside professional groups and community members.
  • Provide orientation and training for Hillcrest staff.
  • Serve as an "on call" Treatment Team member as needed.
  • Obtain/maintain CPR, First Aid
  • Facilitate other residential functions as assigned.
  • Transport residents as needed.
  • Other duties as assigned by CEO or supervisor
ESSENTIAL FUNCTIONS TRUANCY TERMINATION SERVICES:
  • Work with family members to identify reasons for youth's truancy and barriers to regular and positive school attendance.
  • Attend parent/teacher conferences and/or classes with students as needed.
  • Work with school personnel and probation officers to identify solutions and interventions necessary to ensure school attendance, increase youth involvement in school, and improve academic performance.
  • Provide parent education and crisis intervention.
  • Attend initial court hearing as directed by the court to meet with the juvenile and their family; preliminary intake, gathering of basic information, and an overview of the program will be provided.
  • Provide court testimony regarding progress when requested.
  • Maintain close collaboration with referral sources.
  • Provide weekly written reports to the referral source.
  • Provide monthly written progress reports regarding family circumstances and ongoing collaboration/cooperation
  • Monitor school attendance, performance, and behavior.
  • Conduct complete and comprehensive intake and assessment; create a Family Development Plan (FDP) i.e. treatment plan and share with probation officer and school social workers.
  • Complete weekly skills-based classes with youth and parents; individual or group if appropriate.
  • Assess progress of all program graduates and identify youth and families who may benefit from additional training.
  • Assist families with transportation to the program as needed.
  • Other duties as assigned by the CEO or supervisor

This job operates in a residential environment. This role routinely uses standard office equipment such as computers, phones and filing cabinets. This role has the possibility of exposure to inside environmental conditions, such as noise and infectious/communicable diseases, insects, possible exposure to distressed patients, families, or visitors.
WORKING ENVIRONMENT WHILE IN COMMUNITY/HOMES FOR TRUANCY TERMINATION SERVICES
This job operates in a community environment. This role routinely uses standard office equipment such as computers, phones and filing cabinets. This role has the possibility of exposure to adverse weather conditions and unfavorable health and safety conditions such as pets, rodents, insects, unclean environments, foul smells, animal waste, drug paraphernalia, and hostile and threatening individuals.
MINIMUM QUALIFICATIONS :
1. Must hold a bachelor's degree in social work or related human service field.
2. Must have knowledge of relevant clinical issues and demonstrate clinical expertise for populations served.
3. Must meet requirements as identified by State Licensing regulations.
