Position Summary

Cardiovascular Outpatient RN for Heart Failure Clinic: The Registered Nurse, under the direction of the Manager, utilizes knowledge and skills to provide direct age specific and culturally appropriate patient care for congestive heart failure patients using the nursing process based on knowledge of principles of growth and development, serves as a clinical resource, participates in patient and staff education, demonstrates leadership, and serves as a preceptor.

Minimum Requirements

Education

  • Graduate of an accredited nursing program
  • Associates Degree in Nursing

Experience

  • Minimum of 2 years cardiac experience

License/Registration/Certifications

  • Current license to practice Nursing in the State of South Carolina
  • Certification in Basic Life Support
  • Cardiac Rehab only: Advanced Cardiac Life Support

Preferred Requirements

Preferred Education

  • Bachelor’s Degree in Nursing

Preferred Experience

  • 3 or more years cardiac experience

Preferred License/Registration/Certifications

  • Heart Failure Clinic: Certified Heart Failure Nurse Certification

  • Assures continuity of care through effective decision making resulting in quality patient care
  • Documents all aspects of patient care by following established charting procedure to show the patient’s progress or any change in the patient’s condition
  • Consistently follows all regulatory guidelines for the department and hospital.
  • Completes all documentation requirements thoroughly and in a timely manner per hospital requirements.
  • Communicates effectively with all members of the care delivery team.
  • Responsible for participating in process improvement projects within the department.
  • Provides excellent customer service and effective communication with both internal and external customers of the department.
  • Serves as a clinical resource within the department.
  • Ensures that appropriate critical path/standard of care/plan of care is identified and collaboratively implemented.
  • Adhere to and practice appropriate infection control policies and procedures.
  • Perform accurate, legal, and ethical documentation at all times.

Core Job Responsibilities-Heart Failure Clinic Nurse

  • Responsible for review and maintenance of Referral Workqueue
  • Triaging patient calls and electronic messaging from patients regarding signs and symptoms of heart failure.
  • Triaging patient calls regarding medication refills and medication questions.
  • Assists in prior authorization process for medications and non-invasive procedures (echo, stress test)
  • Completes and follow up on patient financial assistance applications
  • Measure and record vital signs, takes patient medical history and chief complaint. Record information accurately in the medical chart.
  • Prepare exam and treatment rooms with necessary supplies.
  • Prepare and maintain supplies and equipment for treatments, including ordering supplies.
  • Assist with scheduling of tests and treatments.
  • Screens, manages, and follows up telephone messages from patients, referrals, and pharmacies.
  • Prepare and send prescriptions electronically per provider’s orders.
  • Performs lab draws, EKGs per provider’s orders.
  • Explains patient’s problems, medications, and procedures to patient.
  • Provide patient education regarding heart failure during patient appointments
  • Teach weekly in person education class to patient and caregivers regarding heart failure
  • Uses printed handouts, booklets, samples, examples, and videos computer programs as applicable.
  • Helps to maintain patient education material. Reviews educational material as needed
  • Assist with inpatient rounding, referrals, education as needed
  • Provide Patient Access Representative (PAR) coverage when needed
  • Dispose of biohazard waste according to state standards.
