Our client seeks a compassionate and resourceful Care Management Specialist to join our remote team in the Miami metropolitan area! This is a 3 months tentative assignment. It is a member-facing role with an average of 3 to 4 visits daily. Mileage will be reimbursed.
Responsibilities:
- Coordinate care for children by assessing healthcare needs, referring to providers, and educating families on benefits.
- Connect families with community resources to address social determinants of health.
- Document outreach, assessments, and care plans in our electronic health record system.
Qualifications:
- Bachelor's degree preferred.
- Pediatric case management experience.
- Experience connecting families/children to local resources (employment, housing, etc.) preferred.
- Valid driving license
- Proficiency in Microsoft Office Suite (Outlook, Word, Excel).
