Job title: Service/Care Coordinator 3Location: REMOTE (Wichita Falls TX – The role would be approximately 75% travel)Duration: 03 Months (possibility to extend)Walk me through the day-to-day responsibilities of this role and a description of the project (Outside of Workday JD):

- SC will have 3 days of visits and 2 days of documentation weekly
- SC will have a caseload and work-assigned tasks
- SC will complete the H2060 assessment
- SC will fill out required forms for each assessment
- SC will make follow-up calls
- Ability to travel 3 days a week 1. Social Service/ Social Work/ Care Management / Services Coordination
- Strong documentation skills 2. Nursing/ LVN/ Medical
