Job title: Service/Care Coordinator 3Location: REMOTE (Wichita Falls TX – The role would be approximately 75% travel)Duration: 03 Months (possibility to extend)Walk me through the day-to-day responsibilities of this role and a description of the project (Outside of Workday JD):
SC will have 3 days of visits and 2 days of documentation weekly
SC will have a caseload and work-assigned tasks
SC will complete the H2060 assessment
SC will fill out required forms for each assessment
Ability to travel 3 days a week 1. Social Service/ Social Work/ Care Management / Services Coordination
Strong documentation skills 2. Nursing/ LVN/ Medical
Education Requirement: Bachelor’s degree in Social Work, Health Services, Behavioral Science or related field, or equivalent experience. Education Preferred: Service Coordination, Case Management, or Nursing experience preferred. Software Skills Required: Microsoft Office, excel Required Certifications: A driver’s license is required.
