Responsibilities:

  • Coordinate discharge plans to ensure patients have a smooth transition from hospital to home or other care setting
  • Assess home situation and psychosocial supports
  • Provide short-term counseling related to the transition from the hospital to home or other care setting:
    • Help patients and families to understand, accept, and follow medical recommendations.
    • Assist with Advance Directives where appropriate
    • Coordinate planning with other members of the care team, including Utilization Review nurses, physicians, and other hospital staff
  • Correctly and timely complete necessary documentation in the medical record to ensure interdisciplinary communication and continuity of care
  • Other related responsibilities:
    • Act as a resource person for other members of the healthcare team regarding community resources
    • Participate in community groups/committees that address the social needs of patients
    • Participate in Spencer Hospital programs such as student shadowing assignments, work with Social Work interns, and other duties as assigned
    • Engages in caring and professional communication

Requirements:

  • Bachelor’s degree in Social Work or related Human Services Field
  • BSW preferred
  • Iowa Social Work license preferred but not required
  • Minimum of 3 to 5 years of acute care experience preferred
  • Able to function independently and demonstrate flexibility, personal integrity and the ability to work effectively with patients, staff, physicians and community resources

Schedule:

  • Full-time - 72 hours per pay period
  • Regular day hours Monday through Friday
  • Hours may fluctuate with workload demands
