Description

JOB TITLE: Physical Therapist

REPORTS TO: Clinical Manager

  1. BASIC PURPOSE

The Physical Therapist provides rehabilitative care (physical therapy) for home health patients needing assistance and training in ambulation, transferring, gait training and home exercise programs. The Physical Therapist works as part of the interdisciplinary home care team including the Registered Nurse, Occupational Therapist, Speech Therapist, Social Worker and Home Health Aide.

  1. PRIMARY RESPONSIBILITIES
  1. Provide physical therapy services to patients according to a Plan of Care. This may include, but is not limited to:
    1. Assess and evaluate therapeutic/rehabilitative/functional status and participate in developing the total plan of care.
    2. Assess for muscle strength, mobility, gait, ROM and transfer capabilities.
    3. Direct physical therapy treatment.
    4. Instruct patients, families and caregivers in the use and care of therapeutic appliances.
    5. Determine priority needs for physical therapy.
    6. Report patients reaction to treatment or changes in care to patients physician as needed and appropriate.
    7. Conducts appropriate evaluative procedures, assesses patients home environment as appropriate, and identifies patients equipment needs.
  2. Initiate physical therapy program and instruct other personnel, patient and/or family/ caregivers in therapy program, as well as the goals of therapy.
  3. Participate in patient case conferences.
  4. Supervises the Physical Therapy Assistant on-site every 30 days or more frequently as needed to guarantee that the Plan of Care is implemented, followed and that patient/ family are satisfied with the care and services being provided.
  5. Participate in Altenheim Home Health Care Home Health Quality Management/ Assessment program as requested.
  6. Confer with the attending physician, home care team members and patient and/or family/caregivers to promote coordinated and comprehensive care.
  7. Participate in meetings and other related activities as needed.
  8. Complete a minimum of twelve (12) hours of inservices per year.
  9. Prepare and submit clinical and progress notes in a timely manner and in accordance with Altenheim Home Health Care, state and federal guidelines and regulations.
  10. Instruct patient and/or family/caregiver and other team members in the patients treatment regimen as indicated.
  11. Meet or exceed delivery of Altenheim Home Health Care service standards in a consistent fashion.
  12. Adhere to Altenheim Home Health Cares Code of Conduct.
  13. Comply with local, state and federal laws, Medicare regulations and established Altenheim Home Health Care policies and procedures.
  14. Interact with all staff in a positive manner, supporting the Altenheim Home Health Care mission.

  1. PHYSICAL REQUIREMENTS (See Attached Physical Requirements Form)
  1. This position requires the physical therapist to work in a variety of environments (housing styles, cleanliness and maintenance, heating and cooling) during visits to patients in their place of residence.
  2. Visual/hearing ability must be sufficient to comprehend written/verbal communication.
  3. The therapist must have ability sufficient to ambulate throughout a patient's home, navigating stairs and other physical barriers typical to private residences.
  4. The therapist must be able to operate a motor vehicle and travel to and visit in patients homes in varying weather and road conditions.
  5. Must be able to deal effectively with stress.
  1. EDUCATION AND EXPERIENCE
  1. Education:
    1. Must possess a bachelors or masters degree in physical therapy approved by an accredited organization: The American Physical Therapy Association, The Council on Medical Education and Hospitals of the American Medical Association, The Council on Medical Education of the American Medical Association and The American Physical Therapy Association.
  2. Experience:
    1. Minimum of two (2) years experience as a physical therapist.
    2. Community/home health experience preferred.
  3. Licensure, Registration, and/or Certification:
  1. Must possess current license as a Physical Therapist from the State of Ohio or
    made application for same.
  2. Must possess valid drivers license.
  3. Must have the use of an automobile in good working condition and automobile insurance.
  4. Satisfactory criminal background check.
  5. Maintains current CPR certification.
