The Patient Navigator provides assistance for patients, families, caregivers and the community to overcome barriers and facilitate timely access to clinical services and resources. 

Duties/Responsibilities:

• Coordinates and facilitates patient appointments, referrals, and follow-up care.
• Educates patients on their healthcare options and helps them navigate the healthcare system.
• Provides emotional support and advocacy for patients and their families, including connecting them with appropriate support services and resources.
• Collaborates with healthcare providers to ensure seamless and efficient patient care.
• Maintains accurate and up-to-date patient records and documentation.
• Assists in developing and implementing employee and patient education programs and resources.
• Serves as a clinical resource to employees through the provision and coordination of patient care.
• Participates in quality improvement initiatives to enhance patient care and outcomes.

Education:
Bachelor’s degree in healthcare administration, social work, or related field, or associate’s degree in nursing required. 

Experience: 
Previous experience in a health care setting required, primary care preferred.

Licensure/Certification: 
May be required based on degree held.
