- Participate in monthly resident pre-OISP team meetings
- Complete and submit annual resident assessment, summary, recommendations, and goals by requested date.
- Attend annual OISP meetings as requested by guardian.
- Evaluates Resident’s goal data and provides suggestions and guidance as needed
- Collaborates with Occupational Therapist on swallowing concerns as needed.
- Collaborates with SLPs within resident school district to ensure continuity of care and goals.
- Assesses and develops guidelines related to Resident hearing status and communication style; ensures care team is adhering to guidelines established to support residents care and safety.
- Completes admission and discharge assessments.
- Provides training to families as needed.
- Maintains confidentiality of all medical information in accord with HIPAA regulations.
- Attends and participates in onsite and offsite meetings, trainings, conferences, and informs relevant staff of pertinent information.
- Performs other duties as assigned
- Applicants must be a minimum of 18 years of age.
- Graduate of an accredited Speech Language Program
- Active unencumbered license in the state of Ohio by the Speech & Hearing Professionals Board
- Current CPR and 1st Aid certified
- Knowledgeable in adaptive equipment and programs specific to those with I/DD.
- Complex Care (tracheostomy and ventilator care) and Developmental Disability field experience preferred.
- Calm, organized, patient, diplomatic, and confident demeanor in working with staff and vendors.
- Comfort in relating to all levels of management and diversity of personalities.
- Advanced attention to detail and accuracy in all work.
- Intermediate ability to prioritize own wide-ranging work load and work independently.
- Ability to maintain the highest levels of confidentiality regarding business and staff information.
- Intermediate ability to communicate both verbally and in writing to a wide range of audiences, including presentation and facilitation skills.
- Exercises good judgment, diplomacy, and perceptual objectivity to effectively and appropriately make decisions.
- Aptitude to define problems, identify trends, collect data, and establish facts to draw valid conclusions.
- Prefer current driver’s license with fewer than 6 points and personal mode of transportation. If not driver, must be accessible and able to report to work when directed and on-call
- Intermediate knowledge of Microsoft Office applications, specifically including Word, PowerPoint and Outlook.
- Knowledge of regulatory compliance and quality management in a health care setting preferred.
- Flexibility of schedule in order to meet operational needs is required.
