Description

Responsible for the delivery of patient-centered, team-based care falling within the scope of practice. Works collaboratively with Nurse Practitioners, RNs, LVNs, Social Workers, Case Managers and Medical Assistants to provide physical and psychosocial assessments, diagnosis and management of chronic and acute health and illness needs. Assumes responsibility and accountability for the continuity of healthcare and health education regardless of the presence or absence of disease. Utilizes standardized procedures and consults the Medical Director in cases outside of scope of practice.

Requirements

Education:
Doctorate in Medicine

License:
Licensed MD/DO in the State of California.

Board Certified Family Practice Physician.

DEA certified.

Experience:
3-5 years of experience providing direct patient care. preferably in an outpatient or community-based setting

Familiarity with working in a FQHC clinic preferred.

Training:
Completion of an accredited residency program in Family Practice.
Knowledge/Skills/Abilities:
Ability to interface effectively with staff, volunteers, and other physicians/providers.
Ability to obtain and maintain credentials at St. Jude Neighborhood Health Centers.

Demonstrates the ability to read, write and speak English and Spanish without an interpreter, preferred. 

Incumbent may travel via personal vehicle throughout the community as well as between other Clinical facilities and departments.
