Summary
  • JOB SUMMARY
    • The RN Care Coordinator will work in partnership with patients, families, nurses, physician, and other qualified health care providers. Coordinates care for patients with chronic disease and manages effective care transitions for them with the continuum. Partners with the provider care team for successful coordination of clinic-based care, diagnostic and preventative testing, and program participation to reduce the severity of chronic disease and avoidable acute illness. Provides effective clinical health coaching to assist patient with self-management of their chronic disease and life-style changes to mitigate health risk. Promotes effective partnerships and utilization of community resources. Facilitates a “shared goal model” within and across settings to achieve coordinated high-quality care that is patient and family-centered and aligned with St. Luke’s goals and objectives.
  • MINIMUM QUALIFICATIONS
    • Education: See Licensure/Certification/Registration requirement.
    • Experience: Two (2) years related experience as a Registered Nurse.
    • Licensure/Certification/Registration: Unencumbered license to practice as a Registered Nurse in the State of Minnesota. BLS Certification with a designation of either BLS Provider or Healthcare Provider from either the American Heart Association or American Red Cross or Military Training Network within ninety (90) days.
  • PREFERRED QUALIFICATIONS
    • Education: Bachelor's Degree in Nursing.
    • Experience: Nursing in an ambulatory care setting, population health management, home care nursing or nurse case manager experience preferred. Experience with electronic medical record is highly desirable. One (1) year experience in motivational interviewing.
    • Licensure/Certification/Registration: Certification and/or licensure in primary training program preferred. Tobacco Cessation certification.
  • KNOWLEDGE, SKILLS AND ABILITIES
    • Knowledge of the current practice standards that apply to practice as a Registered Nurse. Knowledge of equipment commonly used in a family practice setting, Knowledge of medical record documentation, customer and employee relations principles. Ability to use the telephone. Ability to write legibly, see, hear, speak English, and read. Working knowledge of computer processes appropriate to the functional area. Understanding of general health insurance principles and applicable billing guidelines and regulations. Professional verbal and written communication skills are required to establish and maintain effective working relationships with patients, physicians, team members, employees and the public. Ability to use analytical critical thinking and solid problem solving skills.
    • READING - Advanced: Ability to read, analyze, and interpret the most complex documents. Ability to respond effectively to the most sensitive inquires or complaints.
    • WRITING - Intermediate: Ability to write routine reports, correspondence, or procedures.
    • SPEAKING - Intermediate: Ability to effectively present information in one-on-one, small group situations or before groups of customers, clients, and other employees of the organization.
    • MATHEMATICAL SKILLS - Basic Skills: Ability to add and subtract two-digit numbers and to multiply and divide with 10's and 100's. Ability to perform these operations using units of American money and weight measurement, volume, and distance.
    • REASONING ABILITY - Intermediate Skills: Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.

      • Prolonged, extensive or considerable standing/walking; assists with lifting and moving patients, supplies and equipment; considerable reaching, stooping, bending, kneeling, and crouching; lift objects up to twenty-five (25) pounds in weight; works more than eight (8) hours but less than twelve (12) hour shifts.
