The Opportunity:
Saint Lukes North Hospital is seeking a Social Worker to join our team! You will be responsible and accountable for the assessment of psychological, social, emotional, cultural, and financial needs, and the resulting treatment that impacts patients' health and recovery.
Shift Details: On-Call/Weekend/Holiday Rotation. Primarily based at the Barry Road location. Position will have a 2 week rotating schedule: Week 1: Sunday, Monday, Wednesday, Thursday. Week 2: Sunday, Thursday Friday
The Work:
Provides professional social work services that utilize clinical knowledge to address the needs of patients and families during illness, crisis intervention, and post-acute discharge planning.
Collaborates with multidisciplinary team to develop transition plan and address needs in a timely manner.
Participates in program planning and development to meet the needs of customers within the entity, health system and community.
