Shift: Days (Temporary Position)
Hours Per Week:10-15 hours weekly

Hiring Range: $16.00-$21.40

The actual compensation for this position will be determined based on experience and other factors permitted by law.

Responsibilities:

• Responsible for conducting interviews to gather information on clients' social issues and needs.
• Provide guidance for available community support services, including temporary housing, mental health and medical care.
• Assisting with linkages to support services to include making referrals to Health Home Care Management and/or other programs that can provide additional services required by the patient.
• Will collect data to evaluate success with linkages to health home care management.
• Collaborate with emergency department, health homes and behavioral health providers as necessary to assist patients.
• Outreach associate will establish phone/in-person contact with eligible individuals in the emergency department/office as a follow-up to the patient’s discharge from the emergency department. The outreach worker will assist with short term support to patients who are awaiting assignment to a health home care manager and require assistance with linkages to outpatient support and care services.

Education, Training & Experience:

• High school diploma/GED with a minimum of two (2) years of experience in a healthcare setting required, Associate degree preferred.
