St Mary's Healthcare System for Children is Hiring a SPEECH LANGUAGE PATHOLOGIST - Home Care - Queens Near Hyde Park, NY

Brief DescriptionResponsible for the evaluation, planning, implementation, treatment and documentation of patient status for home care patients. Responsible to supervise Speech Language Pathologist Clinical Fellows and cosign their documentation when required. Supports organizations mission to provide the highest quality care by complying with regulatory guidelines, meeting agency standards, and obtaining appropriate certification.
ResponsibilitiesJOB DUTIES / RESPONSIBILITIES:
  • Provides skilled services in speech pathology in accordance with the authorized practitioner's plan of care.
  • Treats the patient through the use of therapeutic activities designed to restore function and improve communication, oral-motor integrity, feeding and swallowing skills, cognitive status, memory and problem solving activities, hearing and speech fluency and quality and all other SLP functions as stated in their practice act.
  • Accepts only patients referred by the Team Supervisor, Case Placement Coordinator or designee.
  • Demonstrates competency in skills required by the agency on at least an annual basis at the time of field evaluation.
  • Evaluates and assesses the patient's level of function by applying diagnostic and prognostic procedures and tests and assists in the development and revision of the plan of care. Assesses the patient's continual care needs. Ensures that the plan of care addresses all problems identified in the assessment or documents rationale for not doing so.
  • Identifies specific augmentative communication needs of patient and refers to appropriate vendor.
  • Observes, records and reports the patient's reaction to treatment and any changes in the patient's condition to the RN or RN Coordinator or Team Supervisor. Reports routinely every 30 days to RN, RN Coordinator or Team Supervisor. Requests referrals for other disciplines as indicated by the needs of the patient or documents rationale for not doing so.
  • Teaches, supervises and counsels the patient, family and other health care team members regarding home rehabilitation procedures, including the home exercise program, and other care needs as appropriate to the patient's condition.
  • Evaluates home safety and coordinates with team members to address issues identified.
  • Coordinates the total plan of care and maintains continuity of patient care by liaising with other health professionals assigned to the same patients.
  • Attends patient care conferences. Initiates patient care conferences for complex and/or multi-disciplinary patients whenever needed to ensure good coordination of care.
  • Develops, prepares and maintains individualized patient care progress records with accuracy, timeliness and according to policies.
  • Submits all visit notes every week or sooner. Submits 60day, D/C summaries and/or quarterly reports as required by the individual program and department.
  • Utilizes agency educational material as appropriate.
  • Meets weekly productivity standards and demonstrates flexibility in scheduling patients as required by department.
  • Cooperates with appropriate staff to provide staff education when requested and arranged by the Team Supervisor.
  • Attends meetings and serves on a variety of patient care and strategic planning committees as assigned by the Team Supervisor or Associate Directors.
  • Participates in the agency's quality improvement program.
  • Attends all staff meetings as required.
RequirementsFormal education required:Master's degree with a major in speech-language pathology.
Holds a certificate of clinical competence and licensure granted by the American Speech and Hearing Association.
Previous Experience RequiredMinimum of one year of experience in speech-language pathology and/or pediatric affiliation.
