What are the responsibilities and job description for the Case Manager - Rosati IRTS position at St. Patrick Center?
The Position:
St.Patrick Center, a ministry of Catholic Charities, is hiring a Case Manager to oversee the daily care of clients in compliance with their identified treatment goals and assist in the implementation of treatment programs. This Case Manager plays an integral role in assisting in the direct implementation of all program goals for Rosati Places' Integrated Residential Program, and related programming. While performing assessments of client's mental illness, substance use, physical health, housing status, and other relative needs in accordance with housing stability this Case Manager should be familiar with a variety of medical diagnoses and treatment plans. As a member of the Rosati Place team, an ideal candidate for this role will have the ability to develop and maintain a positive and therapeutic relationship with each client. The core working hours for this position are 8:00 am to 4:30 pm Monday through Friday. The salary range is between $40,000 an $50,000 a year. Please see full job description for additional details.
