The Position:
In partnership with AmeriCorps, St. Patrick Center, a ministry of Catholic Charities, is pleased to offer a one-year Hospital to Health Housing (H2HH)-Healthcare Outreach Case Manager position. This position is a part of Catholic Charities USA's Healthcare Outreach Program, a national intermediary project, and will work out of the St. Patrick Center location in St. Louis. This is a full time position with the option for part time.

As a member, you will meet virtually with other members serving throughout the country, all focused on assisting individuals within healthcare outreach. The case manager will deliver targeted case management to support improving access to those individuals experiencing homelessness and frequently utilize the hospital systems. The case manager will meet with referrals to the program at St Patrick Center, at partnering hospital, and in the community (local service agencies, shelters, encampments, street). The goal is to improve the health of those experiencing homelessness and decompress the emergency room.

The yearly stipend is $17,600 and a high school diploma or GED is required. The core working hours of St. Patrick Center are Monday through Friday, 8am to 4:30pm.
