Position Summary:
The Patient Care Navigator (Non-Nurse) will serve as the case manager in the Cancer Services Program (CSP) of the Greater Capital Region. The CSP is a NYS funded cancer screening program that provides free breast, cervical and colorectal cancer screening, and diagnostics as needed, to eligible NYS residents who are uninsured. This position will provide care planning, case management and community resource coordination to clients enrolled in the Cancer Services Program who require diagnostic follow up resulting from their breast, cervical and colorectal cancer screenings. This work involves identifying barriers which impact the client receiving timely follow up and ensures the client receive appropriate resources in appropriate settings. This position will also be trained as a Designated Qualified Entity to enroll clients with a cancer diagnosis into the Medicaid Cancer Treatment Program.
Education Requirements:
Bachelor's in Social Work or Human Services required, Master's preferred
Experience Requirements:
1-2 years health care experience highly recommended.
Excellent communication and interpersonal skills.
Advanced knowledge of community resources in cancer.
Must be able to work independently as well as in teams.
Bi-Lingual
Must possess a valid NYS drivers license and be able to travel throughout the 5-county service region.
Pay Range:$21.20 - $28.26
Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
