Job Overview
We are seeking a dedicated Outreach Specialist to join our team. The ideal candidate will play a crucial role in community engagement, program development, and outreach initiatives.

Responsibilities
· Canvas, conduct face to face interactions, engage, and triage those people experiencing homelessness that are disengaged from community service to quickly connect them with a range of community services

· Collect personal I.D. and documentation, income verification, homelessness verification to include obtaining 3rd party verification as required and disability verification, when applicable, and upload them into HMIS.

· Ensure all clients are added to the By Name List (BNL) and work on housing plan

· Support clients in obtaining income through workforce development referrals, or connection to a Social Security Income/Social Security Disability Income Outreach, Access, and Recovery (SOAR) specialist.

· Conduct CAN assessments and provide ongoing case management services

· Refer clients to local drop-in centers

· Enter program enrollment, assessment, case notes by completing the case note template HMIS and Smartsheet’s, when applicable, within the required timeline

· Attend & participate in mandatory training, staff meetings and CAN meetings

Qualifications
- Associates or Bachelors Degree in related field

· Min. 2 years of human/social service experience, supervisory experience preferred

· Well organized, detail oriented, and possess critical thinking skills
