POSITION SUMMARY
Our Patient Navigator will provide housing location, navigation, Discharge Planning, Transitional Care Management, and stabilization services to homeless transition youth and adults who have been identified through the Managed Care Plans Eligibility Profile. They will be responsible for assessing needs and connecting clients to resources that is appropriate for their needs. This may include (but not be limited to) crisis/bridge housing, permanent supportive housing, shared housing, or general affordable housing. The Patient Navigator will interface with landlords, Hospital Discharge Planner, accompany (In person, Virtual or Phone) clients to housing appointments, and assist clients throughout the entire housing search and move-in process. Once the client is in housing, the Patient Navigator will assist them in developing an individualized action plan, with a focus on housing stability, and will connect youth to community-based services to meet their ongoing needs.

ESSENTIAL DUTIES & RESPONSIBILITIES

Work with staff to ensure that each client completes an initial intake, needs assessment,

Determines nature of member's situation by interviewing member; assessing medical, psychological, emotional, and social information

Establishes course of action by exploring options; setting goals with member.

Obtains assistance for member by referring him/her to community resources; arranging for appointments; establishing rapport with other agencies.

Fosters member's action or adjustment by interpreting attitudes and patterns of behavior; explaining and pointing out new options.

Maintains record of case by documenting member's situation and member's own actions.

Evaluates member’s progress against care plans, goals, interventions and modifications as needed.

Communicates and coordinate services with member and member’s care team, such as Primary Care Physician (PCP), specialists, medical providers, and ancillary providers as necessary to meet member’s medical and psychosocial needs.

Facilitate member access to behavioral health, social services and community based services as needed.

Actively participate in Interdisciplinary Care Team Meetings.

Assist youth in determining personal goals and plans to achieve those goals through the development of an Individual Action Plan (IAP).

Provide housing stabilization services to client, which may include topics like money management, life skills, accessing transportation, understanding their lease, self-advocacy and other topics based on the needs of clients.

Partner on and participate in outreach efforts to clients experiencing homelessness

Establish and cultivate relationships with landlords and other housing providers to build a portfolio of housing options for clients.

Enter data in required databases, compile and submit reports that track client status and support services provided.

Maintain accurate client case files of all required eligibility documents.

Attend staff development training as assigned.

MINIMUM QUALIFICATIONS

1. 1-2 years’ experience or related work and a Bachelors-level Degree; or an
Associates-level Degree and 5 or more years’ experience providing housing- based support services to young people facing multiple barriers to housing stability.

2. Experience working with youth and Adults experiencing homelessness and housing instability, and/or have had experience in foster care, probation, mental health systems.

3. Reliable, flexible team player who works well with minimal supervision, who has a good work ethic, and an ability to set and maintain personal boundaries.

4. Demonstrated ability to work on multiple projects simultaneously, and to meet deadlines.

5. Proficient in Microsoft Word, Excel, and PowerPoint

All candidates are subject to a criminal history check and meet CRCD’s criteria regarding criminal history.

Job Type: Full-time

Pay: From $50,000.00-75,000 per year
