What are the responsibilities and job description for the Supervisor of Care Navigation Services position at St. Vincent's Hospital Westchester?
Under the direction of the Project Coordinator, the Supervisor of Care Navigation Services has direct oversight and supervisory responsibility for all care navigation and peer services. This includes ensuring that clients are 1) appropriately linked to primary care providers, and are receiving needed medical treatment; 2) receiving the support and guidance needed to successfully navigate transitions of care, especially for clients considered high-risk and in urgent need of rapid engagement and treatment, and 3) receiving the guidance and support needed to access services associated with health related social needs: lack of housing, financial & food insecurity, legal/forensic entanglements, gaps in education, and other psycho-social stressors that impact clients' overall health.
Qualifications for position: NY State licensed behavioral health clinician, with significant experience in case management and care navigation. Must have at least 3 years of direct service experience, and at least 1 year in a supervisory capacity.
Skills and knowledge required: This position requires knowledge of integrated care coordination services in behavioral health settings, and the ability to work collaboratively with multiple providers across various service systems in support of clients' whole-health recovery and well-being. Additionally, the position requires the ability to provide leadership and guidance for all members of the Care Navigation team, and to help develop and utilize tracking methods to ensure that clients' needs and service goals are being met.
Salary range: $70,000 - $80,000
Hours per day or week: 37.5 hours weekly, generally Monday - Friday; some after-hours responsibilities required.
