JOB
An employee in this classification provides professional social work services to Social Services clients in the Community Alternatives Program (CAP). and other Family Services. This position provides case management services to disabled adults and their families. The social worker provides services to assist clients and their families in understanding their medical diagnoses and treatment plan and to adapt and cope with their problems. This worker completes an initial assessment to evaluate activities of daily living, instrumental activities of daily living, social environment, economic resources, and home environment. Worker also completes a cognitive, social, mental, and physical evaluation in sometimes complicated and/or questionable cases to fully and accurately assess client needs and determine the best possible treatment and intervention plan. A treatment plan is developed to meet the unmet needs of the client. The worker arranges for services and coordinates and interacts closely with other service providers to carry out the treatment plan. Position must keep records and completes ongoing documentation. Worker is responsible for establishing and developing interdisciplinary and multidisciplinary teams designed to address the problems of and meet the unmet needs of the client. Worker works closely with other disciplines to assure efficient and appropriate delivery of quality client services. This position is responsible for the Crisis Energy programs and some guardianship cases. This position also performs other duties as assigned.
EXAMPLE OF DUTIES
Provides professional social services to Social Service CAP clients.Reviews patient charts and interviews patients to determine needs; determines eligibility for CAP services; completes required forms; verifies Medicaid eligibility; establishes and maintains accurate and complete client records. Prepares, implements and monitors patient care plans, making changes when necessary.Reviews monthly and daily task sheets for providers.Refers patients to other department divisions and outside agencies for services as appropriate; assists clients in accessing needed services.Reviews billings for CAP office; monitors care costs and makes changes in care plan as necessary to ensure compliance with cost limitations.Conducts home visits to monitor progress and ensure clients are receiving necessary assistance.Attends staff meetings to discuss and coordinate cases; communicates with nursing staff, physicians and others to coordinate patient care.Conducts case audits.Receives and responds to inquiries, concerns and complaints regarding assigned programs and projects.Participates in community awareness efforts to inform the public of CAP services.Identifies community programs and services available for client assistance.Gathers information for and prepares various reports as required by the County and/or other agencies.Operates a vehicle and a variety of office equipment, including a computer, printer, copier, telephone, calculator, etc.; uses clerical and computer supplies.Interacts and communicates with the immediate supervisor, other County department heads and employees, clients and family members, physicians, nurses and other health professionals, various outside social service / health agencies, and the general public.Attends meetings, training, workshops, conferences, etc., as required to enhance job knowledge and skills.Performs general administrative/office work as required, including attending meetings, preparing reports and correspondence, entering and retrieving computer data, sending and receiving faxes, copying and filing documents, reviewing correspondence, answering the telephone, etc.Performs related duties as assigned.
SUPPLEMENTAL INFORMATION
Bachelor’s degree in social work from an accredited school of social work; Bachelor’s degree in a human services field from an accredited college or university and one year directly related experience; Bachelor’s degree from an accredited college or university and two years directly related experience.Those with a Social Worker degree from an accredited school of social work will be given preference; however, anyone with a Human Services Degree or a 4-year degree may apply as “work against appointments” and will be considered.
