We are onboarding new employees using a hybrid approach. The Department supports flexible work arrangements. Information about the Case Management and Quality Performance Division:

The Case Management and Quality Performance Division provides the lead role in policy decisions and technical assistance for intake, eligibility, quality, and case management functions. The Division works to ensure quality compliance and performance excellence by monitoring, trending, training, and providing technical assistance to our case management agencies and stakeholders. The Division is responsible for case management delivery and quality performance monitoring to assure a cohesive, efficient process and structure needed to optimize the experience of the individuals served.

Description of Job

What you'll be doing:

  • Serving as the staff authority on the development, implementation, aggregation, analysis, and reporting of Case Management Stabilization within the Office of Community Living. Information generated from this position is submitted to the HCPF executive leadership to determine overall stabilization of the case management ecosystem.
  • Developing internal and external data requests that are crucial to influence OCL case management staff, benefits and services administrators, and senior management on trends and issues regarding Case Management Agency (CMA) performance and activities to achieve stabilization related to CMRD transitions, CCM implementation and PHE unwind.
  • Requesting and analyzing data from CMAs to evaluate CMA stability and determining whether CMA is at risk of not meeting contractual requirements.
  • Scheduling and facilitating meetings with CMAs to review CMA data, discussing CMA requirements and activities towards CMA stabilization, evaluating CMA current action plans for achieving stability and the reported data to determine if stability is occurring and will be achieved.
  • Managing CMA meeting notes and providing summaries to CMAs and leadership.
  • Developing reports and managing dashboards on CMA performance towards stabilization.
  • Developing comprehensive knowledge and use of the Care and Case Management(CCM) system to assist Case Management Agencies with system functionality and works internally with HCPF and vendors on improving CCM for end users.

MINIMUM QUALIFICATIONS:

There are two ways to qualify for this position:

Option 1:
Experience Only:
Seven (7) years of related professional experience providing case management or direct support services in the Long Term Services and Supports or Home and Community Based Waivers service system and 3 years experience developing and implementing public policy or administering public programs.

Option 2:
Education and Experience:
A bachelors degree from an accredited institution in Human Services, Psychology, Education, Public Policy, Social Work or a related field and three (3) years of professional experience providing case management or direct support services in the Long Term Services and Supports or Home and Community Based Waivers service system and/or 3 years experience developing and implementing public policy or administering public programs.

Appropriate education will substitute for the required experience on a year-for-year basis. Relevant education will be calculated as the following:
  • An associates will count as two years of experience
  • A bachelors will count as four years of experience
  • A masters degree will count for six years of experience

  • A doctorate or juris doctorate degree will count for seven years of experience


PREFERRED QUALIFICATIONS:
  • Prior state experience related to the duties of this position
  • Experience developing and implementing quality assurance measures
  • Project Management experience
  • Experience with the Department’s Care & Case Management system
DEFINITION OF PROFESSIONAL EXPERIENCE: Work that involves exercising discretion, analytical skill, judgment, personal accountability, and responsibility for creating, developing, integrating, applying, and sharing an organized body of knowledge that characteristically is uniquely acquired through an intense education or training regimen at a recognized college or university; equivalent to the curriculum requirements for a bachelor's or higher degree with major study in or pertinent to the specialized field; and continuously studied to explore, extend, and use additional discoveries, interpretations, and application and to improve data, materials, equipment, applications and methods.
CONDITIONS OF EMPLOYMENT:
  • All positions at HCPF are security sensitive positions and require that the individuals undergo a criminal record background check as a condition of employment.
  • Employees who have been disciplinary terminated, resigned in lieu of disciplinary termination, or negotiated their termination from the State of Colorado must disclose this information on the application.
