Total Compensation Estimator Tool Your Specific ResponsibilitiesThis position involves the coordination and provision of HIV and other STD education and testing program services to incarcerated inmates within the local county correctional facility and other detention and/or prison facilities authorized to receive such services by the Florida Department of Health, Division of Disease Control Jail Linkage Program. This position reports directly to the Operations and Management Consultant II under HIV Prevention, Position No. 61273
Performs direct HIV/STD Counseling and Testing Services on inmates submitting and consenting to have such tests performed in accordance with Florida Department of Health HIV/STI Testing Guidelines and processes specimen collections for submission to the State Laboratory. Performs direct Court Ordered HIV/STI/HCV counseling and testing services when required. Acts as the Liaison between the Florida Department of Health in St. Lucie County and Detention Facility Officials and subcontractors providing medical and other ancillary services within the detention facility for the purpose of coordinating services and ensuring that Jail Link Programmatic Services, needs, and goals are provided at the highest level of quality and accessible to all inmates where applicable. Attends all meetings with respect to service coordination and special event functions concerning the Jail Link Program and maintains a continuous up-to-date contact information list of detention center and Subcontractor personnel. Facilitates regular HIV/STI/HCV prevention education classes for inmates of the detention facility and distributes and collects facilitator evaluation forms and knowledge-based HIV/STI/HCV pre and post-tests at the completion of each class. Awards certificates of completion to inmates participating in HIV/STD/HCV prevention classes. Completes required Jail Link Monthly and Quarterly Reports accurately and within the time frame requested for submission. Completes records and reports including case management forms, laboratory request forms, and travel reimbursement forms, etc. in accordance with current CHD, HIV/AIDS Linkage to Care - Prevention Program and STD program policy and/or procedures. Duties as Assigned Performs case management activities as directed in the process of discovering and/or identifying individuals infected with a STI, hepatitis, HIV/AIDS or other bacterial, biological and/or viral agent. These activities will include but not limited to: Performing Original Post-Test Counseling for Positive Results and Partner Counseling, testing and Referral Services; Work prioritization and records documentation and management, clustering, HIV pre and posttest counseling, visual case analysis and epidemiological techniques, Linkage Module/STARS data entry and/or other related activities.
On identification of confirmatory HIV/STI/HCV positive test results performs original post-test counseling interviews and partner counseling, testing and referral services, and links individuals testing positive into the care system both internally within the detention facility and externally upon release in collaboration with the Linkage to Care Program. Communicates with public and private clinics, clinicians, hospitals, laboratories, and other health care providers to obtain demographic, medical, or other specific client information including address, telephone number, place of employment, social security number, race, sex, date of birth, test results, diagnosis, treatment, etc. and/or to arrange medical evaluations and treatments for a client.
The incumbent is also expected to conduct counseling and testing in the community and in a clinic setting in Area 15 (St Lucie, Martin, Okeechobee, Indian River Counties). The incumbent is responsible for assisting with linkage/re-engagement activities and referrals for the area by ensuring individuals are linked to care within 30 days and that linkage to care (LTC) field records in the HIV/AIDS Linkage Module and STARS are followed up on and closed out within timely manner. This position will assist with Data-to Care linkage records and assure linkages & referrals to medical, social services & treatment services. Identify newly diagnosed clients, re-engages client who have fallen out of care, and assist providers, in locating non-compliant clients and bring them back into care and treatment. Ensure all newly diagnosed individuals are linked to care within 30 days. Ensures previously diagnosed are re-engaged into care within timelines, coordinate with local Regional Linkage to Care lead (RLC), HIV/AIDS surveillance and STD program staff to ensure Data to Care (D2C), Molecular Monitoring Project (MMP), Molecular HIV Surveillance (MHS), and Ryan White Re-engagement line list are worked in a timely manner and complete the D2C collection form to appropriate staff. The incumbent is responsible to identify barriers to care and assist HIV -positive individuals with necessary referrals and/or practical aspects of adherence such as transportation, obtaining needed medications, receiving Medicaid or private insurance, etc. Performs other duties as assigned including outreach events and community services.
Participates in clinic, community, or impromptu screenings activities by performing HIV/STIs/Hepatitis venipuncture and/or using other rapid testing techniques. Collaborates with other DOH or community partners to provide outreach counseling and testing education/presentations at all community events including detention facilities. Ensure complete and accurate processing of specimens and data collected. May act as an outreach lead. Prepares and maintain timely and accurate records & reports according to program guidelines and requirements. Responsible for fostering and initiating a client led support group for those clients wishing to participate in a peer led monthly activity and assist with recruitment. Utilize collaborative agencies for buy in, adverstising and assistance. Attend montly community/staff meeting. Provide feedback to supervisor and providers through debriefing sessions. Develops appropriate materials, resources, and processes to improve the outcome and efficiency of quality care for HIV positive clients, high-risk clients, CBOs, and LTC staff. Participates in and disseminates information on trainings increase efficiency of program.
Maintains client confidentiality and security in accordance DOHP50-10 Inf1ormation Security & Privacy Policy Confidential Information. Records, certifies, and submits EARs, Cell Phone verification, Everbridge Alerts, CSRs and timesheets per DOH policy within specified policy time frames. Participates in department and unit meetings. Performs other related duties as assigned or when required. Ensures the accuracy and completion of HIV/STD counseling and testing forms per state guidelines. Submits logs, final list of responsive external providers, businesses, appropriate organizations/venues, and monthly monitoring reports to immediate supervisor. Participates in local/ statewide, meetings, training and local committee, staff, and unit meetings.
Required Knowledge, Skills, And AbilitiesMust be able to travel in and out of county within area 15, must a valid DL, case manager experience, phlebotomy training. Self-motivated, self-starter willing to take initiative, flexible, ability in PowerPoint presentations skills, exceptional interpersonal skills, efficient in communication, good organizational and time management skills, ability to work in teams, Able to complete Linkage to Care (ARTAS) and Disease Intervention Specialist training, experience with delegating tasks, must be comfortable with talking to individuals and groups. Knowledge and experience in working with diverse communities. Ability to plan and coordinate work. assignments. Experienced using Microsoft Office (Word, PowerPoint, Excel, Outlook).
Qualifications Minimum:
  • Willing to become Certified in Phlebotomy and complete training on Disease Intervention Specialist and Linkage to Care ARTAS training
  • Willing to travel out of county, undesirable neighborhoods, and/or between counties.
  • Willing to work before and afterhours and during weekends.
  • One-year case manager experience and/or client education and counseling and/or Social Work.
  • Valid driver’s license
  • Willing to provide services to all residents in detention facilities/jails, rehabilitation/drug addiction facilities, mental health facilities, and/or subcontractors in Area 15
  • Florida Department of Health is required to work before, during, and/or beyond normal work hours or days in the event of an emergency. Emergency duty required of the incumbent includes working in special needs shelter or performing other emergency duties including but not limited to, response to or threats involving any disaster or threat of disaster man-made or natural.
Preferred
  • Experience in a Lead Role
  • Certified phlebotomist and/or trained phlebotomist
  • Experience working with HIV/STD programs and/or related work.
  • Experience performing Quality Assurance, performing file reviews and/or develop processes and reports.
  • Experience working with detention facilities, drug rehab facilities, mental health facilities and/or related work.
  • Experience with Microsoft word, Excel, PowerPoint, and other programs.
  • Case management experience, patient care coordinator/navigation and/or experience with caseloads.
  • Experience of providing risk reduction education on topics such as HIV, STDs, safe sex practices, and/or conducting community events.
  • Experience working with marginalized, low-income, and disenfranchised communities.
  • Bilingual (English/Spanish and/or English/Creole)
