Job Responsibilities
Under general supervision, provide direct nursing care to clients exclusively enrolled in the Ryan White program which includes: screening patients for eligibility, medical and non-medical case management, including treatment adherence, evaluation, counseling and providing a broad range of services that link clients to sub-specialty services. Responsible for case management intake assessments and performing comprehensive assessment of health care needs.
Duties:
Fulfill nurse protocol training requirements in HIV, STD, Immunization, and other relevant areas to function as an expanded role PHN.
Provide direct nursing care including physical assessments, education, treatment, and develop and document a plan of care consistent with the clients’ level of need.
Screen and refer clients to other community service providers as needed. Assist with referrals and follow-up treatment planning as indicated.
Provide sexual health promotion and prevention education, including counseling clients and family regarding the importance of medication adherence for viral suppression.
Participate in staff meetings and assist with developing and implementing quality improvement projects as needed.
Maintain accurate medical records, documenting is a must, assist with data entry and quality assurance monitoring.
