Southwest District Health (SWDH) is recruiting a Resources & Services Navigator (Case Manager) to join the dedicated group of public health professionals in their Pre-prosecution Diversion Program within the Division of Family and Clinic Services.

As a Case Manager and public health employee, you will serve an active role in improving the health of your community. As part of a small, but mighty team, you will provide case management services to individuals participating in the Pre-prosecution Diversion Program in coordination with the Canyon County Prosecuting Attorney’s Office. This role will empower clients to achieve their goals, promote their well-being, and advocate for their needs within the broader community. Individuals participating in this program will have unmanaged behavioral health conditions and are at risk of entering or repeatedly entering deeper into the justice system.

This is a limited-service position through June 2026 and may be extended as funding allows. Length of Limited-Service appointments are defined as appointments tied to funding for a specific program, grant or project that is not indefinite in nature.

This position may include a teleworking option from within Idaho.

Our generous benefits package elevates the total compensation for a full-time Resources & Services Navigator (Case Manager). Our offering transforms the hourly pay range from $22.76 - $26.78/hour to an estimated $34.44 - $39.36/hour, showcasing the value we invest in your well-being and professional growth.

Typical Duties:

Candidates must be able to complete the essential job functions with or without reasonable accommodation and meet all the mandatory qualifications of the position.

  • Conduct interviews and review enrollment forms to understand participants' strengths and service requirements, identifying additional assessments as needed.
  • Connect participants with appropriate community resources to meet their identified needs, including medical care, legal assistance, housing, and employment.
  • Develop interim case plans in collaboration with participants, setting achievable goals and tracking progress to ensure completion.
  • Introduce participants to available department services, community resources, and the assessment process, while also minimizing risks through appropriate referrals and continuous support during case management.
  • Guide participants through the completion of required documents, explaining relevant laws, policies, and procedures to ensure accuracy and compliance.
  • Refer cases to cross-program teams for development of an integrated service plan and ongoing case management
  • Monitor client progress, recording achievements and agreements in detailed progress notes for ongoing tracking.
  • Report on the performance of individuals to Canyon County and applicable community partners.

Minimum Qualifications:

You must possess all the minimum qualifications listed below to qualify for this position. All required experience and/or education must be listed under the work history section of this application OR on your resume and must include a detailed explanation of your roles and responsibilities at each relevant employer.

This position requires incumbents not related to a Canyon County Commissioner or other Canyon County elected Officials by blood or marriage within the second degree of kindred.

Minimum Qualifications (MQ):

  • Good knowledge of Human services principles and practices, typically gained through either a closely related bachelor's degree, 2-4 upper division college courses related to the subject and 2-4 years of mid-level work experience closely related to the subject, or a combination of coursework and experience that represents the knowledge level above.
  • Approximately 2 years of related work experience in the following areas:
    • In case management

MQ Specialty:

  • Valid Driver’s License

Ideal Knowledge, Skill, and Abilities:

  • Bachelor's degree in social work, psychology, counseling, or a related field.
  • Previous work experience in case management, social services, or related fields.
  • Strong interpersonal and communication skills.
  • Knowledge of community resources and social service systems.
  • Knowledge of human behavior and social environment.
  • Knowledge of substance use, biological, and social effects.
  • Knowledge of Idaho’s justice system.
  • Ability to work independently and as part of a team.
  • Ability to communicate effectively with a variety of individuals including program colleagues, judges, prosecutors, legal defenders, case managers, peer support specialists, treatment providers and client population.
  • Commitment to upholding ethical standards and maintaining client confidentiality.
  • Skill in working with resistant and justice involved treatment populations.
  • Skill in de-escalating confrontation situations.
  • Ability to develop and maintain effective working relationships.
  • Fluency in speaking, writing, and/or reading both English and Spanish.
