Building 10, Transitional Housing Program

Our team has an exciting opportunity for a registered nurse with an interest in medical recuperative care (MRC)! This role focuses on providing care for vulnerable and high-risk Veteran patients at the Transitional Housing Program (Building 10) in Port Orchard, Washington.

The Medical Recuperative Care Nurse (RN2) provides education to MRC clients and participates in care planning and medication monitoring. This position is ideal for registered nurses with at least one year of experience facilitating discharge plans and continuous care planning activities.

Please Note:
This role is a part-time position with an expected end date of April 1st, 2029. The schedule for this position will vary, with a minimum of 10 hours per workweek; hours will increase based on client needs and care plan. Schedule will be revised on a routine basis in response to client needs. Local travel within Washington state may be required for meetings, trainings, and other events.
  • This position has been posted within the assigned pay range through Step T. Step U has not been published as this is considered the longevity step, in alignment with state HR compensation rules.
Position(s) at this location will also receive 5% Premium Pay for facilities open twenty four hours per day. Employees must physically work at the facility 3 days per workweek to qualify. The 5% Premium Pay is in effect until June 29th, 2025.

Duties

Some of what you'll be doing:

  • Reviewing discharge paperwork to include treatment plans, medication lists, and diagnosis list.
  • Working with VA Liaison and DVA MRC case manager to develop and implement care plan.
  • Participating in monthly care reviews and ah-hoc case review meetings.
  • Immediately reporting significant changes in client status, medication intake, or overall condition to VA Liaison and DVA MRC case manager.
  • Working with VA Liaison and DVA MRC case manager to modify care plan as needed.
  • Submitting required VA documentation to the VA Liaison.
  • Reviewing discharge documentation for complete understanding of MRC client's medication needs.
  • Reviewing medication list with MRC client and providing education on medication types and administration.
  • Completing and maintaining medication-monitoring documentation.
  • Measuring and recording vitals for MRC clients based on care plan.
  • Assessing clients to determine if emergency medical interventions are needed.

Qualifications

Required:

  • Current Washington State registered nurse license OR unencumbered registered nurse multistate license.
  • One (1) year of experience facilitating discharge plans and continuous care-planning activities.
Preferred/Desired:
  • Experience working with
    • Veteran populations.
    • Individuals experiencing homelessness with a broad understanding of the barriers individuals may face in obtaining permanent housing and potential solutions.
Special Requirements:
  • Must be able pass a DSHS BCCU Background Check prior to beginning services and every two (2) years in accordance with WDVA Policy 615.000 Background Checks. Information from the background check will not necessarily preclude employment but will be considered in determining the applicant's suitability and competence to perform in the position.
  • Must comply with annual Tuberculin Screening in accordance with DVA policy 670.000 Tuberculosis Testing.
  • Requires medical evaluation, fit testing, training, and use of respiratory protection as required by the Washington Industrial Safety and Act standards. Must be able to wear a respirator, medical gloves, gowns, eye protection, and other personal protection as required by a job or task.
  • Must possess and maintain a valid driver's license.
  • Candidates who are offered a job with WDVA must possess work authorization which does not require sponsorship by the employer for a visa now or in the future.
