Your Responsibilities for the RN Case Manager - Home Health position:
- As our new RN Case Manager - Home Health you will carry a caseload of 1-6 patients per day or 15 per week, visiting your patients in their homes.
- Reporting to one of our Home Health Clinical Managers, you will provide clinical services according to company and state regulatory practices.
- Ideally, you reside in SJ or within a short commutable distance to efficiently carry a patient caseload for our South Team.
Your skills as our new RN Case Manager - Home Health :
- Current or obtainable CA RN license.
- At least 1 year of prior work experience as a Home Health or Hospice RNCM is much preferred. Related clinical work experience will be seriously considered.
- We offer comprehensive Home Health training, as required.
- A related college healthcare degree is preferred, or commensurate experience.
