The role of the Care Supervisor in Waynesboro, GA

Job Description : As a Care Supervisor, your responsibilities include :

• Managing and coordinating care needs reported by case managers
• Ensuring updated schedules, continuity of care, and delivering quality care by the clinical care team
• Facilitating case conferences and follow-up actions related to meetings
• Mentoring and coaching clinicians and CSS role, addressing development needs, and holding them accountable to KPIs
• Handling voluntary attrition, providing support and finding solutions to personnel concerns
• Supervising and directing the work of team members, including assigning visits, scheduling, and ensuring timely completion
• Managing the site in the absence of the Clinical Manager, Executive Director, or Administrator
• Overseeing clinical, administrative, and recordkeeping functions
• Responding to phone inquiries in a timely, professional, and compassionate manner
• Participating in on-call nursing schedule and providing skilled bedside nursing care in patients' homes

Requirements of Qualified Candidates :

• Must be a registered nurse with a current nursing license in the practicing state
• CPR certification is mandatory
• Preferred experience in a home care setting with at least two years of management or supervisory experience
• Proven ability to work within an interdisciplinary setting
• Must be a licensed driver with an insured and working automobile
• Knowledge of professional standards, Medicare Conditions of Participation, and regulatory requirements
• Excellent observation, nursing judgment, and communication skills
