Patient Services Coordinator
Job Description : In the role of Patient Services Coordinator, your responsibilities will include :
- Coordinating and communicating care needs as reported by case managers
- Updating schedules and ensuring continuity of care for patients
- Conducting case conferences and follow-up actions
- Providing coaching and mentorship to clinicians and CSS role
- Supporting team development and addressing personnel concerns
- Supervising and directing team members' work and schedules
- Managing the site in the absence of the Clinical Manager or Executive Director or Administrator
- Overseeing clinical, administrative, and recordkeeping functions
- Responding to phone referral / inquiries professionally and compassionately
- Participating in on-call nursing schedule and providing direct skilled bedside nursing care
Requirements of Qualified Candidates :
- Registered nurse with current licensure
- CPR certification
- Previous home care experience with management or supervisory experience preferred
- Ability to work within an interdisciplinary setting
- Valid driver's license and insured automobile in good working order
- Knowledge of professional standards, Medicare Conditions of Participation, and regulatory requirements
- Excellent observation, nursing judgment, and communication skills
