Kindness is not just a tagline used in a phrase, it is our mission. Suburban Hospice is seeking a Care Coordinator that will reflect that mission and ensure a seamless transition for patients upon referral to hospice care. The Care Coordinator services Hancock, Henry, Rush and Shelby counties. 

The Care Coordinator is responsible for coordinating the referral-to-admission process for hospice patients.

• Promotes agency services in the community through marketing and public relations efforts to maintain existing business and identify opportunities for business growth
• Builds rapport and maintains relationships with referral sources to maintain and grow a strong referral network
• Provides administrative support to the admission team to ensure that all eligible patients receive care, barriers to admissions are addressed and resolved and agency growth and profitability is maximized
• Coordinates and facilitates referrals to Hospice care based on Hospice admission criteria
• Obtains and documents referral information including current medication and treatments, physician, patient and primary care persons agreement to Hospice evaluation visit.
• Evaluates patient referrals to determine appropriateness for admission to the Hospice Program.
• Supports and maintains a culture of safety and quality.
• Conducts in-home/facility assessments in accordance with Suburban Hospice care admissions policies and procedures.
• Acts as a Suburban Hospice representative at institutional and community programs and functions.
• Notifies Hospice Team of patient’s acceptance for service and provides intake data, essential medical background, preliminary care plan, and medical orders.
• Collaborates with clinical management and staff to ensure that the needs of patients are being met.
• Attends meetings with hospital case management, discharge planning and IDG meetings to ensure quality patient care transition
• Meets with patients pending discharge from facilities for pre-admission screening and introduction to agency services
• Responsible for meeting agency goals set forth by expected business development in the region
• May perform other duties as assigned.
