Summit Community Care Clinic is Hiring an Immunization Coordinator Near Frisco, CO

Position SummaryThe Immunization Coordinator will coordinate with Summit County Public Health, SBHC MA Immunization Champions, and Medical Assistant Supervisor for reconciliations, orders, and other items as needed related to the State Immunization program. They perform related duties as assigned by the supervisor. The Immunization Coordinator will provide safe, effective, and quality primary, preventive, and evidence-based health care to patients in a culturally competent manner, focusing on health promotion and disease prevention. The Immunization Program Coordinator provides quality, cost-effective, and compassionate medical care, and treatment of the defined patient population, with care being as comprehensive as possible within the limitations of the Care Clinic.
Essential Functions
  • Implements and coordinates all aspects of SCCC's and SBHC's Immunization program, including compliance with the Colorado Department of Public Health and Environment's requirements. This includes receiving, storing, organizing, monitoring temperatures, and ordering and administering vaccines, as well as ongoing continuing education for medical assistants, nurses, and providers regarding immunizations.
  • Provides clinical support to medical assistants, health navigators, front desk staff, nurses, and providers.
  • Consistently, accurately, and objectively documents patient interactions and other information in the patient's electronic health record. Charting is accurate, timely, and appropriate.
  • Focuses on quality customer service for all patients, coworkers, volunteers, and partners outside SCCC.
  • Establishes and maintains relationships with vaccine vendors as appropriate.
  • Participates in the Clinic's QI/QA Program as directed by supervisor. This may include collecting, reporting, and/or reviewing quality data and participation in performance improvement projects.
  • Takes the lead on reporting and recording any vaccine excursions and leads the investigation of unanticipated outcomes regarding vaccine errors or events.
  • Provides clinical and non-clinical in-services for SCCC staff.
  • Assists Clinical Manager and Chief Medical Officer with projects as assigned.
  • Maintains compliance with Occupational Safety and Health Administration regulations. Responsible for maintaining knowledge of such regulations and for the application of such guidelines in the workplace.
  • Maintains all aspects of patient confidentiality according to HIPAA requirements.
  • Refers and consults with other professionals as needed.
  • Educates patients and family members about health care issues.
  • Attends Staff Meetings and/or reads the minutes.
  • Performs all duties as described in the primary role.
  • Trains all new medical assistants to ensure the skills checklist is completed before they can perform any immunizations.
  • These duties may change, and additional duties may be assigned.
Additional Requirements
  • All jobs at the Care Clinic are impacted by various grants which are necessary for our sustainability. Serving in this role has direct responsibilities to those grants and is vital to the success and continuation of those grants.
  • This position may be scheduled Monday – Saturday.
  • Complete all required training as designated by HR and the position supervisor.
  • Adhere to all policies within the organization and those outlined within the SCCC Employee Handbook.
  • Comply with all SCCC Vaccine Mandates.
  • Willingness to travel and work at other SCCC locations as determined by the supervisor.
  • Comply with all applicable Credentialing and Privileging requirements timely.
  • Embrace the SCCC Mission and Vision and agree to work within the SCCC Guiding Principles and the principles set by the position supervisor.

Education/ExperienceMust Haves to be successful:
  • Qualified CA, MA, LPN, BSN, NP, PA, MD, or DO with at least two years' experience in a clinical setting or its equivalent. Previous vaccine program management experience preferred.
  • Bachelor’s Degree or equivalent.
  • CPR certification.
  • Exhibit cultural competence and exceptional teamwork skills.
  • Ability to read, write, and clearly speak the English language.
  • Ability to read, write, and clearly speak the Spanish language preferred.
  • Ability to interpret a variety of instructions and deal with multiple linguistic and cultural variables.
  • Ability to relate cooperatively and constructively with patients, co-workers, administration, physicians and providers, community agencies, referral sources, regulators, and other health team members.
  • Ability to enlist the cooperation of others.
  • High tolerance for stress.
  • Excellent organizational and leadership abilities.
  • Outstanding communication and people skills.
  • Knowledge of the industry’s legal rules and guidelines.

  • Previous non-profit and/or volunteer experience.
  • Ability to clearly speak both English and Spanish preferred (bilingual).
Supervisory Responsibility: Lead Immunization team meetings at least monthly
