Are you an experienced RN seeking an exciting opportunity in home health? We're hiring a compassionate RN for a 13-week assignment in St. Augustine. If you have recent home health experience, proficiency in OASIS and Homecare Homebase (HCHB), and a genuine commitment to patient care, we want to hear from you!
Your Role:

• Collaborate with healthcare professionals to develop personalized care plans for patients.
• Document patient assessments and progress using OASIS and HCHB systems.
• Administer medication and provide wound care as needed.
• Ensure accurate documentation and reporting using OASIS and HCHB.

Requirements:

• Active RN licensure in Florida.
• Minimum one year of recent home health experience.
• Proficiency in OASIS and Homecare Homebase (HCHB) systems.
• Current BLS Certification from the American Heart Association.
• Previous experience managing a home-to-home workload preferred.
