Job Description:

As a Home Health RN, you will be responsible for delivering high-quality nursing care to patients in their homes. Your duties will include, but are not limited to:

• Conducting thorough patient assessments and developing individualized care plans
• Administering medications and treatments as prescribed
• Monitoring patient progress and making necessary adjustments to care plans
• Educating patients and their families on disease management, medication administration, and self-care techniques
• Completing and submitting OASIS documentation with a 24-hour turnaround time
• Collaborating with the interdisciplinary healthcare team to ensure comprehensive patient care

Requirements:

• Experience: 1-2 years of recent home health experience
• Technical Skills: Proficiency with Homecare Homebase (HCHB)
• Documentation: Absolute proficiency and 24-hour turnaround time with OASIS documentation
• Licensing: Licensed to practice nursing in New York (NY)
• Other Requirements:
  • Valid drivers license and reliable transportation with current car insurance
  • Compliance with all Joint Commission requirements
