Job Description:

Sunbelt Staffing is currently seeking a qualified Registered Nurse (RN) for a contract position in home health care in Hammond, LA. The ideal candidate will have 1-2 years of experience in home health, proficiency in Homecare Homebase as a charting system, and a commitment to providing exceptional patient care.

Responsibilities:

• Provide skilled nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies.
• Perform comprehensive assessments of patients' health status, develop individualized care plans, and implement appropriate interventions.
• Administer medications, treatments, and procedures as ordered by the physician and ensure accurate documentation of all care provided.
• Complete OASIS documentation with a 24-hour turnaround time and maintain accurate and up-to-date patient records using Homecare Homebase.
• Collaborate with interdisciplinary team members, including physicians, therapists, and other healthcare professionals, to ensure coordinated and holistic patient care.
• Educate patients and their families on disease management, medication administration, and home safety practices.
• Adhere to ethical and professional standards of nursing practice and maintain confidentiality of patient information at all times.

Requirements:

• Valid Registered Nurse (RN) license in the state of Louisiana.
• Minimum of 1-2 years of experience in home health care as an RN.
• Proficiency in Homecare Homebase as a charting system is required.
• Ability to complete 25-30 productivity units each week, with compensation for a full 40 hours of work.
• Availability for Monday-Friday schedule, with flexibility for weekend work or on-call as needed.
• Reliable transportation, valid driver's license, and active car insurance.
• Ability to pass a background check, drug screen, and provide all necessary medical documents in compliance with the Joint Commission standards.
