PARAPROFESSIONAL (PP)/(AP)

Service: Psychosocial Rehabilitation

Summary: The Psychosocial Rehabilitation Program provides skill development activities, educational services, and pre-vocational training for adults and seniors. This service is for transitional and supported employment services to individuals with severe and persistent mental illness. These services are designed primarily to serve individuals who have impaired role functioning that adversely affects at least two of the following: employment, management of financial affairs, ability to procure needed public support services, appropriateness of social behavior, or activities of daily living. Assistance is also provided to clients in organizing and developing their strengths and in establishing peer groups and community relationships.

Minimum Position Requirements:

  • All Paraprofessionals providing Psychosocial Rehabilitation services shall have a minimum of one (1) year documented experience with the adult mental health population; personal care and/or working with individuals with developmental disabilities and/or substance use disorders.
  • Minimum requirement of education must be a GED or high school diploma.
  • Ability to follow/create lesson plans for skills improvements
  • Excellent communication skills, both oral and written, including proficiency in progress note writing.

Program Responsibilities:

  • Abides by approved working schedule from 9:00 a.m. to 5:00 p.m. (Monday through Friday)
  • Provides skills development activities to include: (1) community living - such as housekeeping, shopping, cooking, use of transportation facilities, money management; (2) personal care – such as health care, medication management and grooming; (3) social relationships and (4) use of leisure time
  • Provides interventions and services to promote the highest possible outcomes through restoration and maintenance
  • Prepares classroom lessons and facilitates groups to PSR clients in a professional manner
  • Participates in Adult Treatment Team Meetings
  • Reports and submits Incident Reports to PSR Program Director or Qualified Professional and QA/QI Director
  • Transports PSR clients on outings – ONLY with evidence of current operating drivers’ license
  • Accompanies PSR Program Director/Qualified professional on outings by monitoring and supervising clients
  • Adheres to the agency’s internal policies and procedures
  • Ensures the PSR Program adheres to the Managed Care Organizations (MCOs) and NC DHHS facility regulations and Medicaid guidelines
  • Receives approval from Program Director how to assist clients to appropriately work on person-centered goals in their PCPs
  • Document billable services rendered and submit according to Records Management and Documentation Manual Division regulations and agency’s policies/procedures
  • Ensures documentation standards are met; (document discussions with guardians, case managers, natural supports or other parties involved with the treatment of PSR clients
  • Attend staff meetings, trainings and off-site meetings as deemed necessary
  • Advocates for client rights
  • Manages clients at risk behavior(s)
  • Conduct and input notations of consumers on a daily basis
  • Performs other duties, tasks and assignments delegated by PSR Program Director/Qualified Professional/Associate Professional, Clinical Director, QA/QI Director and/or CEO.
