Job Summary:
The Department of Obstetrics and Gynecology at SUNY Downstate Health Sciences University is seeking a full-time Community Relations Assistant. The successful candidate will:
  • Work closely with both the Obstetrics and Gynecology as well as the Pediatrics Department.
  • Practice to engage patients with the purpose and focus on education, assessment and referral to the program.
  • Support practices and social work to ensure patients and staff are aware of the program.
  • Serve as the persistent on-site presence and linkage to co-located Community Relations Assistants.
  • Work with the program's Social Worker to ensure a closed loop of communication with practice patients and connect patients to the program.
  • Engage with patients to ensure that practice patients are educated and are aware of the services.
  • Support practice staff in the implementation of the program.
  • Perform screening and assessment.
  • Provide linkage and oversight of co-located Community Relations Assistant to ensure proper implementation and adherence to institutional practices and procedures.
  • Meet regularly with the Brooklyn Network (BPN) staff to ensure closed loop and efficient implementation.
  • Assist in maintaining the files and data base.
  • Where appropriate, outreach to educate and inform about the program.
Required Qualifications:
  • High School Diploma or equivalent.
  • 2 years of applicable or equivalent experience.
  • Proficient in Microsoft Office.
  • Excellent communicator with willingness to learn and collaborate with others.
  • Demonstrate strong work ethics.
Preferred Qualifications:

Monday to Friday; 8:00am to 4:00pm

Commensurate with experience and qualifications
